Jessica's family were farewelled at a ceremony at St Mary's Catholic Church in Concord last Wednesday while she remained in a coma.

Former Home and Away actress Jessica Falkholt has died, three weeks after the horror Boxing Day crash that killed her parents and sister.

"The family have asked for privacy during this very hard time".

Jessica Falkholt was travelling with her parents and sister along the NSW coast when they were involved in a collision with another vehicle.

The 29-year-old had been in a critical condition in St George Hospital after being pulled from the wreckage on December 26 the NSW South Coast along with her 21-year-old sister Annabelle, who later died.

Ms Falkholt and her sister Annabelle were pulled from the crash, as the vehicles went up in flames, but her parents died in the burning auto, along with the 4WD's driver.

The family had been driving home to Sydney after spending Christmas together at Ulladulla when a Toyota Prado driven by methadone-affected driver Craig Whittal crossed onto the other side of the road.

Ms Falkholt played Hope Morrison in 16 episodes of the long-running TV soap opera Home and Away, and is the star of a soon-to-be released Australian film titled Harmony.
